You can travel with a friend without adding them to your AllClear policy. Your AllClear policy only insures the people named on your policy schedule.

Who’s insured: Only the people named on the policy schedule are insured under the policy.
If you want cancellation/curtailment protection for your friend because of your medical condition: On Gold, Gold Plus and Platinum, Travelling Companion cancellation cover can provide cover for your travelling companion’s cancellation and cutting the trip short if they are insured under another travel insurance provider and your pre-existing medical condition (accepted in writing) causes the cancellation. This only applies if the additional premium has been paid and is shown on your policy schedule.

If you’ve paid for Travelling Companion cancellation cover, it can cover your travelling companion’s cancellation and cutting the trip short when they are insured elsewhere and your accepted pre-existing medical condition causes the cancellation (and it’s shown on your schedule).
